What you need to know for your appointment

All appointments are scheduled through our main clinic number at (830) 890-5181. We accept most private insurance plans, Medicare, and Care Credit but please call for verification.

For all clinic visits please bring:

• Your insurance card(s)
• A photo ID
• A referral if one is required by your insurer
• Check, credit card (Visa or MasterCard accepted), or cash for payment

To expedite registration at your first appointment, we ask that you complete a few pieces of paperwork before arrival. These forms may be downloaded here or they can be mailed to your home address.
