All new patient appointments are scheduled by calling our main clinic at (830) 890-5181.
Your new patient forms will be sent to you by text through our HIPAA secure communication system called KLARA. Please fill out/submit your forms before you arrive for your appointment.
If you are not able to complete your forms on your phone or tablet, you may download and print out your paperwork here.
Bring your referral if one is required by your insurer.
